Comprehensive assessments identify needs, create a personalized action plan, and track progress with guidance from your Community Navigator. Access care through any of our community partners, who can enroll you in Hope Grande Prairie to get started.

SELF REFERRAL

If you can’t contact our community partners directly, you can self-refer to Hope Grande Prairie. Share your needs and story, and our team will respond within 48 hours (on a business day).

LIBRARY CONNECTION

Visit us at the Grande Prairie Public Library during our drop-in times—we aim to be there Tuesdays and Thursdays to offer support, answer questions, and help connect you to services in a safe, welcoming space.
