Getting connected with Hope Grande Prairie is simple and designed to meet you where you’re at.

 

You can start by reaching out to a community partner you’re already connected with, or by completing a self-referral through our online form. If you’re working with a partner, they’ll talk with you about your needs and guide you through the process. If you self-refer, a member of our Hope Grande Prairie team will connect with you to learn more, answer your questions, and walk you through next steps.

At the center of Hope Grande Prairie is HopeHub—a secure, web-based platform that supports the backbone of our coordinated support system. HopeHub allows service providers to work together from one shared profile, making it easy to track progress, share updates, and stay connected.

Once you’re enrolled in Hope Grande Prairie, you’ll use the platform to complete an assessment that looks at 13 key areas of wellbeing—called the “pillars of health”—including Housing, Recovery, Employment, Mental Health, Physical Health, Safety, Transportation, Education, Dental, Food, Childcare, Legal Assistance, and Financial Stability. This assessment gives a full picture of where things are at and where support may be needed.

From there, a personalized Action Plan is automatically developed in HopeHub, identifying each pillar as Crisis, At Risk, or Thriving—creating a clear and shared roadmap forward.

Together, you and your Navigator will determine which supports make the most sense for you by setting tasks for items in your Action Plan. The right service providers are brought together as part of your care team, working collaboratively to help you achieve your goals.

Support doesn’t stop after those initial connections. Your team continues to work alongside you, adjusting as your needs change and making sure you feel supported every step of the way.
