Below is an overview of the programs and services offered, including how each functions and the various pathways to access care through Hope Grande Prairie.

INFORMATION SESSION
Learn how Hope Grande Prairie works, what supports are available, and whether it’s the right fit for your organization to join as a partner. In this 1-hour session, we’ll walk through the program, its history, what day-to-day participation looks like, and the features HopeHub offers.

INITIAL ASSESSMENT & REASSESSMENT
The initial comprehensive assessment identifies needs across key areas of wellbeing and creates a personalized action plan. Our Community Navigator will guide you through the process, helping set goals and track progress. Follow-up assessments are conducted at least every 30 days to monitor progress, adjust supports, and ensure ongoing, coordinated care.
Accessing care is available through any of our community partners. When a need is identified through conversation, our partners can enroll you in Hope Grande Prairie to get started.
SELF REFERRAL
If you encounter barriers that prevent direct contact with our community partners, you may choose to self-refer to Hope Grande Prairie. When self-referring, you can outline your needs and share your story. Our team will respond within 48 hours (on a business day) to discuss the next steps.

Visit us at the Grande Prairie Public Library for support in a safe, welcoming, and low-barrier space:
We aim to be available during the following times:
Tuesdays: 10:00am – 1:00pm
Thursdays: 1:00pm – 4:00pm
While we do our best to be there consistently, occasional changes may occur.
